Claims Process: Funeral Plan​

Acumen Funeral provides affordable, innovative and unique value-added products within their standard funeral covers. Acumen Funeral will provide you and your family with a well-balanced funeral solution that will meet your needs and requirements.

Documents Required

  • The claim form, together with the following documents, must be returned to Acumen Funeral within 30 days of the event
  • Original certified copy of the Identity Document or Birth Certificate of the Claimant / Beneficiary
  • Original certified copy of the Identity Document or Birth Certificate of the Deceased person
  • Original certified copy of the Death Certificate of the Deceased person
  • A certified copy of the registration of death form (DHA 1663)
  • Copy of the bank statement or cancelled cheque of the bank account to be used for the payment of the proceeds
  • Medical Practitioners Report in the event of claiming for a stillborn child or an accident
  • Copy of the Police report in case of unnatural death (Accident, murder etc.)
